Strengthening the general health condition of the population in rural areas, promotion of health care and empowering rural communities
The health reforms led by the Ministry of Health have put a strong emphasis on strengthening the primary care system through health promotion among population, thus increasing their responsibility for their own health. Acknowledging the relevance of health promotion, SDC has been supporting health promotion through the project “Community Action for Health”since 1999. The Ministry of Health has accepted the model developed by the project and has incorporated its expansion throughout the country into the health reform plan.
The Goal of the exit phase is to fully integrate and anchor the CAH model into the existing public health care system, thus ensuring its sustainability and continuity. The underlying impact hypothesis is that a sustainable health promotion system, which is publicly supported and implemented by a nationwide network of public and community health promotion organizations at all the levels, will continue to contribute to the improvement of health outcomes in the country.

The CAH model is fully integrated with the public health care system It is expected that by 2017 almost all VHCs will have reached a level of organizational development allowing them to sustain without support of this project. The project will strive to shift the costs for the key health actions to the MoH with which previous agreements have been developed on the shift of financing.
Health awareness and behaviour of population in rural and urban areas is improved. The project will conduct health actions in collaboration with the Republican Centre for Health Promotion (RCHP) in the Ministry of Health, and will invest in tools and information material for the VHCs. As part of the exit strategy, the gradual reduction of investments by the project in the area will be increasingly covered by the Ministry of Health.
